These forms can also be accessed from G:\UGALibs\PERS\Evaluation. As you enter information into the performance evaluation forms, please do not save the file to the G:\UGALibs\ drive, which would result in the information being available for all Libaries personnel to view.
Remember, leaving the form open without saving prevents others from accessing, so please save the forms you need as soon as possible.
